You must fill out the RSVP form if you are attending the Fall Banquet.
Note: If you have a guest, the guest must also fill out the RSVP form.
On the RVSP, you will indicate whether or not you have purchased a ticket or intend to use points to pay for your ticket. Additionally, this form includes meal selection. Please make sure to include any allergies that you may have at the very end of this form.
